What is Saesalchim?

Depressed scars are associated with fibrous tissue pulling the skin inward from within.

On the surface, the skin appears indented, but in reality, beneath the dermis layer,

hardened tissue, like fibrous cords, is strongly adhering to the scar area, pulling it downwards.

It is expected that the skin will rise back to its original height only after these adhered fibrous tissues are separated.

The procedure that breaks these adhesions is precisely Saesalchim, which is subcision.

Principles of Saesalchim Treatment

1. Severing Adhered Tissue (Release)

Fibrous adhesion bands located deep within the skin

are carefully separated using a special needle or cannula.

Through this process, the adhered structure of the scar, which was pulled downwards, is released, helping the depressed area naturally rise.

2. Inducing Micro-Bleeding (Regeneration Signal)

The subtle bleeding that occurs during the process of separating adhered tissues

can act as an important signal to promote collagen synthesis.

This stimulates fibroblast activity, laying the groundwork for new skin tissue to regenerate in the scar area.

3. Improving Blood Flow and Promoting Tissue Regeneration

If necessary, microneedling treatment and PDRN (DNA injection) are applied together

When can Saesalchim treatment be considered?

What is Multi-Saesalchim? (A treatment that combines existing Saesalchim with the Er:YAG shouldering technique)

This is an improved treatment method that combines typical Saesalchim (subcision) with the shouldering technique.

When considering scar treatment, simply separating the adhered tissue beneath the scar may not be sufficient.

Even if adhesions are released with subcision, scars with clear, deeply depressed borders may not regenerate a completely smooth skin surface.

Our skin tends to recover damaged areas with a height and texture similar to the surrounding tissue.

However, if the scar borders are abruptly cut off, this natural healing process may not proceed smoothly.

Therefore, the border areas of the scar are delicately refined to be gentle (shouldering).

This process reduces the height difference with the surrounding skin, allowing the scar area, when regenerating, to blend more naturally with the adjacent skin and appear less prominent to the naked eye.

Indeed, one of the reasons why satisfactory improvement is not seen with general Saesalchim treatment alone is deeply related to this point.

Even if the adhesions beneath the scar are resolved, if a distinct border remains, it can hinder skin regeneration. 

Therefore, shouldering can be considered a meaningful process that can complement the limitations of existing treatment methods for depressed scars with clear borders. 

Types of cases where Multi-Saesalchim can be considered

1) Those who have not experienced significant improvement with general Saesalchim or other scar treatments

Even after receiving subcision or laser treatment, there are those who feel their scars have not improved as much as expected.

This may be because simply releasing the adhesions at the bottom of the scar makes it difficult to induce natural skin regeneration up to the steep scar margins.

In such cases, by applying shoulder treatment and gently refining the margins, better scar improvement can be expected.

The second type is scars that are deeply indented with distinct, steep margins. 

For example, in the case of steeply indented scars like Boxcar scars, even if skin tissue regenerates from within,

the lateral margins remain firm like a wall, hindering the complete healing process.

By gently refining the margins through Er:YAG shoulder treatment, it helps reduce the height difference with the surrounding skin, allowing the scar to blend more naturally.

The third group consists of those who wish to experience scar improvement with fewer sessions than regular Saesalchim (new skin acupuncture) treatment.

Traditional Saesalchim can be expected to show gradual improvement over multiple sessions. In contrast, multi-Saesalchim approaches scar care by simultaneously treating the adhered bottom, steep side margins, and surface.
